The Office of the Registrar maintains the official name and address for all students. Changes of name
require completion of a
special form, available in the Office of the Registrar or you may print one from the web. Please
note that this form is used for a variety of purposes, so only change those fields necessary. Changes
of address are not processed on the same form. For information on how to change your address, visit our

If you are or have ever been employed by Indiana University, even as a work-study employee, please
provide documentation, such as a photocopy of your drivers license. If you are or were an IU employee
and your marital status has changed, please provide a copy of appropriate documentation. This will be
used in adjusting any employee benefits. Contact Human Resources with any questions.
