2007 Staff Annual Reviews


Staff annual review forms are intended to be completed by individual staff members (self assessment by all employees) and given to their supervisor for review.

Bi-Weekly Staff
Permanent Part-Time Staff
  • Bi-weekly staff and permanent part-time staff should submit their completed annual review form to their supervisor, Director, or Division Head by Monday, February 11, 2008. Please click <here> to access the appropriate form.
Professional Staff
  • Professional staff should submit their completed annual review form to their supervisor, Director, or Division Head by Monday, February 11, 2008. Please click <here> to access the appropriate form.

  • Professional staff who report directly to the Vice Chancellor and Dean should submit their completed annual review form to the Vice Chancellor and Dean’s office by Monday, February 11, 2008.

    • Annual review forms for your staff are due in the Vice Chancellor’s office on Monday, March 10, 2008. Since completed forms of your staff members are due in your office on Monday, February 11, this will provide you with four weeks to review the forms and meet with each staff member before they are due in the Vice Chancellor and Dean’s office. Completed annual review forms should contain signatures of the staff member, immediate supervisor (if applicable), and the Director.
Note to Division Heads

Your staff should submit their completed annual review forms to you no later than Monday, February 11, 2008. This will provide you with four weeks to review the forms and meet with each staff member before the forms are due in the Vice Chancellor and Dean’s office on Monday, March 10, 2008. Completed forms should contain signatures of the staff member, immediate supervisor (if applicable), and the Division Head.
