Division Heads & Academic Program Directors
  • Supervisors will be provided with links by e-mail to access and complete annual review documents.  Completed documents will be saved and submitted through SharePoint. 
  • A hard copy of the self-evaluation form and the supervisory form should be signed by the employee and supervisor at the review meeting.   
  • Original signed documents should be routed to Personnel Administration.  They may be placed in our mailbox located in the Division of Administration and Finance or hand-delivered to LC Room 2108.

Performance Evaluations

IUPUC staff members complete self-assessments each year to note their accomplishments and overall performance. The assessments include a review of current job duties and responsibilities. Performance evaluations are not conducted for student employees.

Annual Review Process for 2013

The annual review forms for 2013 will be processed through SharePoint. Directions for completing the forms, along with the personalized link to your annual review form, will be distributed via e-mail.

2013 Self-Evaluation

  • Timeline: Forms will be distributed to staff the week of Monday, January 13, 2014. 
  • Due date: Forms must be completed by all staff employees by Monday, February 10, 2014.

2013 Supervisory Forms

  • Timeline: Forms will be distributed Monday, February 10, 2014.
  • Due date: Forms must be completed by Monday, March 10, 2014.

The self-evaluation and supervisory review processes should be complete no later than Friday, March 14, 2014.
