Division Head & Program Director Reviews

Division heads and program directors complete administrative review forms each year to note their accomplishments and set goals for the coming year. The 2014 forms can be downloaded from the appropriate link below.

Note to division heads:
Division heads should submit an original, signed 2014 program director administrative review form for each of their program directors to Associate Dean for Academic Affairs Gary Felsten by Monday, February 23, 2015. 

Division Head or committee evaluations of faculty FARs should be submitted to the Office Personnel Administration for personnel files.

  

Performance Evaluations

IUPUC staff members complete self-assessments each year to note their accomplishments and set goals for the coming year. For 2014, staff will complete a self-evaluation and review a listing of their job duties and responsibilities. Both forms should be given to your supervisor by February 9, 2015, so they can be reviewed prior to the completion of the supervisory evaluation and review meeting. Please note performance evaluations are not conducted for student employees at this time.

    *A separate, personalized Word document containing current job duties and responsibilities will be sent to staff in January.  Please review the document, noting any changes, and give it to your supervisor along with your self-evaluation form.

Staff Supervisory Evaluation Forms

Due date to the Office of Personnel Administration: Monday, March 9, 2015

Supervisors will receive two forms from each employee as indicated above.  Please review these documents and download the appropriate supervisory form below. 

Review meetings should be scheduled so original, signed documents can be forwarded to the personnel administration by March 9, 2015:

  • Supervisory evaluation
