2015 Faculty Annual Reports

Division Head & Program Director Reviews

Division heads and program directors complete administrative review forms each year to note their accomplishments and set goals for the coming year. The 2015 forms can be downloaded from the appropriate link below.

Note to division heads:

Division heads should submit the following completed items to Associate Dean for Academic Affairs Gary Felsten by Monday, February 22, 2016:

  • Narrative evaluations of all full-time faculty prepared by division committees, including updated CVs
  • 2015 Program Director Administrative review forms
  • Division Head Administrative review form

All faculty review information submitted to Dr. Felsten will be forwarded to Vice Chancellor and Dean Richards for review by Monday, February 29, and to personnel administration by Monday, March 7, 2016.

*This timeline is necessary to correspond with the 2016-17 budget construction process.

  

2015 Performance Evaluations

Annual performance evaluations have been conducted for IUPUC staff for several years and will continue through 2015.  Beginning January 2016, performance evaluations will be conducted three times per year.  The performance evaluation document has been updated and shortened to be less time-consuming and cumbersome for all and to accommodate the more frequent reviews in 2016.  The single document will be submitted for all staff regardless of rank or full-time/part-time status, and it can be accessed in Word or pdf format at the links below. It should be completed primarily by the supervisor, but time should be allowed for the employee to complete two required sections for comments and goals. This single document replaces all documents used in the past.  Please note performance evaluations are not conducted for student employees at this time

2015 staff performance evaluations are due to the Office of Personnel Administration no later than Monday, March 7, 2016.

2016 Performance Evaluations Schedule

Beginning January 2016, performance evaluations will be conducted three times per year as follows:

Review period Due dates
January-April Friday, May 20, 2016
May-August Friday, September 23, 2016
September-December Friday, January 27, 2017

The purpose of more frequent performance evaluations is to provide timely and useful feedback to our staff.  Our goal is to provide supervisors and employees with opportunities to have short, regular dialogues with their employees that are less structured and more relaxed vs. the typical annual review meetings in the past.  Annual reviews are time-consuming for supervisors and employees and many times the information is not relevant. The new review document and shorter, more frequent meetings will require far less time and work for everyone and at the same time provide our employees with useful information in a timely manner. 

Documents for each four month review period will be added to the webpage in Word and pdf formats as they are needed.
