Division Heads and Program Directors complete administrative review forms each year to note their accomplishments and set goals for the coming year. The 2016 forms can be downloaded from the appropriate link below.

Note to Division Heads:
Division Heads should submit the following completed items to Associate Dean for Academic Affairs Gary Felsten by Monday, February 20, 2017:
- Narrative evaluations of all full-time faculty prepared by division committees, including updated CVs
- 2016 Program Director Administrative review forms
- Division Head Administrative review form
All faculty review information submitted to Dr. Felsten will be forwarded to Vice Chancellor and Dean Reinhold Hill for review by Monday, February 27, and to Personnel Administration by Monday, March 6, 2016.
*This timeline is necessary to correspond with the 2017-18 budget construction process.
