Division Heads and Program Directors complete administrative review forms each year to note their accomplishments and set goals for the coming year. The 2016 forms can be downloaded from the appropriate link below.

Note to Division Heads:

Division Heads should submit the following completed items to Associate Dean for Academic Affairs Gary Felsten by Monday, February 20, 2017:

  • Narrative evaluations of all full-time faculty prepared by division committees, including updated CVs
  • 2016 Program Director Administrative review forms
  • Division Head Administrative review form

All faculty review information submitted to Dr. Felsten will be forwarded to Vice Chancellor and Dean Reinhold Hill for review by Monday, February 27, and to Personnel Administration by Monday, March 6, 2016.

*This timeline is necessary to correspond with the 2017-18 budget construction process.


2016 Performance Evaluations

Third Reporting Period of 2016: September–December

Review forms for the third reporting period of 2016 (September–December) are due to the Office of Personnel Administration on Friday, January 27, 2017.  Forms can be accessed below:

2016 Performance Evaluations Schedule

Performance evaluations for 2016 will be conducted three times as follows:

Review period Due dates
January-April Friday, May 20, 2016
May-August Friday, September 23, 2016
September-December Friday, January 27, 2017

The purpose of more frequent performance evaluations is to provide timely and useful feedback to our staff.  Our goal is to provide supervisors and employees with opportunities to have short, regular dialogues with their employees that are less structured and more relaxed vs. the typical annual review meetings in the past.  Annual reviews are time-consuming for supervisors and employees and many times the information is not relevant. The new review document and shorter, more frequent meetings will require far less time and work for everyone and at the same time provide our employees with useful information in a timely manner.
Documents for the next review period will be added to the webpage in Word and pdf formats as they are needed.
