If you are an IUPUC student, faculty, staff member, or visitor and have an accident, injury, or illness requiring immediate medical treatment when on campus, you will be asked to download and complete an incident report form as soon as you are able to do so.
Once you have completed and signed the appropriate form, please submit it in person or by U.S. mail to:
